You would think it should be an easy question to answer. Unfortunately, it is not and is one of the biggest problems with our current health system. Let's take a shoulder replacement surgery as an example. When a patient has surgery, his insurance company will be accountable to the surgeon, anesthesiologist and hospital. Each of them is a separate charge. The amount paid can actually vary by the insurance company depending on the deal they negotiated. Anthem could pay less than United that pays more than Aetna, etc. As a general estimate of the balls stadium, a replacement shoulder surgery will result in the hospital being paid 20k and the surgeon about $1,500. The patient can then pay a portion of that burden depending on his particular insurance plan and his deductible and co-paid. If a patient has a 10% co-payment and maximum deductible of 5k per year, his surgery would cost them approximately 2,500. If your deductible max for the year is $1k, you could pay $1k. There are some interesting points that are often misunderstood as the difference between charges and payments. A hospital can "charge" anything they want for a service, but they only receive the payment that has been negotiated with the insurance company. For example, they can charge $100.000 for a shoulder replacement but when the hospital sits to negotiate with the insurance company, they will agree on a payment of about 20k. I often read articles in newspapers and magazines about the high cost of health in the USA and they like to quote things like a Tylenol cost $50. Unfortunately, that's not a precise presentation. A hospital can "load" fifty dollars for the Tylenol but that is not what is being paid, and in many cases nothing is paid for the Tylenol because it is included in the general fee. For example, if you have a shoulder replacement in Virginia Beach the hospital is not paid for every Tylenol, antibiotic and chicken dinner. In most cases, they are paid a flat rate independently. A second point is that most of the spending on medical care goes to hospitals not the doctor. Shoulder replacement surgery is a good example of that. The surgeon receives approximately $1,500 while the hospital receives about $20,000. In the future, efforts to reduce the cost of orthopaedic surgery procedures will focus on moving surgeries outside hospitals and in surgery centers. The cost of a shoulder replacement in a surgery center can reduce the charges by almost half. In the past this was not a big problem for patients because their insurance company paid most of the bill, but now with high deductibles and co-paid plans these charges are being transferred to patients. As a result, there is now demand from consumers for cheaper options and the market is responding. So when I ask your doctor, "How much will my surgery cost?", don't be surprised with the blank look. The question is hard to answer without asking many other questions such as: what is your insurance, what is your deductible and co-paid, you are doing surgery at the hospital or surgery center, ....... and the list continues.
